Dear all, I wish to share a case of 48 yrs old male patient with no comorbities, who presented to us with moderate to severe covid disease. He had a 60% lung involvement, with a ct score of 16/25 on hrct thorax. The patient had self medicated for first seven days of fever despite being positive. On eighth day he came to us with a dropping oxygen saturation of 89%. We started him on nasal oxygen with NRBM mask at 4 litres, and his saturation came up to 97%. We planned to use a new drug called 2DG (2 deoxy glucose), which is a new molecule made by DRDO labs, India. This is a cheat glucose molecule wherein the position of oxygen is changed to 2nd carbon in the hexagon. The virus particles which use this 2DG molecule for their biochemical needs are rendered non functional and they cannot infect other cells. Thereby the infection is controlled and the patient goes off oxygen soon. This shortens ICU stay as well. We gave our patient 2DG 90mg/kg/day in oral form. Along with this he was given remdesivir and other standard of care medicines. On day three he was off oxygen and shifted out of the ICU. He was then discharged to home after finishing six doses of remdesivir. The results with 2DG were outstanding. Routinely such patients end up spending 7 to 10 days in ICU with gradually increasing oxygen requirement. Many of them eventually need bipap support and ventilator in later days. This molecule has come as a miracle molecule to save these patients. Pro, cons and cost: The molecule is used only for those with moderate to severe covid on oxygen. It has not been tried on patients on NIV and ventilators. The molecule is marketed by dr reddys lab and mankind pharma. Each sachet cost approx 900. For an adult above 52 kg weight, we need 4 sachets per day for ten days or till discharge whichever is early.
